Reading something from CDC but from many other examples, one of the consequences of conflict, internal turmoil and radical isolationism, all of which human-settled space in Star*Drive has seen in abundance, is the persistence and spread of disease.

They might be never-tamed illnesses brought out from Earth, dangers of exotic worlds or, maybe most likely, bio-weapons from a number of eras of conflict or even from before human arrival on a planet.

There's been plenty of opportunity for infections to develop, but perhaps not as much for them to spread across the Stellar Ring. One vector might have been the incursions of a large but ill-organised ground force. The Thuldan legions covered a wide area, but they seem likely to have dealt with such problems ruthlessly and effectively.

An artificial infective agent may not have been designed primarily to harm the subjects, but to transform them in some way. It may not be working as designed.

Colonising nations (the Concord, other stellar nations, local multi-system powers), religious groups and non-governmental charities might send medical assistance, with various complications as we see in our own time.

edit: Such assistance could take the form of biokinetic healing, which might work, pseudoscientific theories, which won't, any better than they do in our own PL, or trial remedies, rejected batches or discontinued products from large corporations, for which the hapless locals take their chances.

I wondered about (2500 AD) Ground Ratings C and D, but they don't correspond well to the nations I might suspect of uncontrolled carriage of disease in the late war years: say, Hatires, Nariacs and Solars.
